We appreciate your help! Insurance verified /obtained pre-authorization Received medical clearance from PCP Consents signed and received Latex or other allergies noted Patient payment received Patient information sent to surgery center Proper tissue type ordered and confirmed by MD Confirm required patient information; diagnosis, patient ID, DOB all correctly entered for order Click here to log into your surgeon’s OWSA account for easy ordering access: https://tissue.oneworldsightalliance.org/member/login.aspx If you do not have direct access to OWSA please use the attached updated form. Suggested confirmations 1 week prior and day prior to surgery: Confirmed surgery date is booked, date, time, and location Patient has confirmed transportation to and from procedure Reminder not to eat/drink prior after midnight Rx’s sent to pharmacy and received / patient aware when to start drops and/or medication Patient has received the Path to Sight brochure and Lions World Vision Institute wristband which will serve as a reminder of honoring the Gift of Sight.
